Exceptional Circumstances Suspension Request

Request for Credit/Suspension

Parents may apply for a credit or have their child's fees suspended for an extended period due to medical or extenuating circumstances. These reasons may include surgery, broken limb, family bereavement etc. Medical suspension requests may also extend to the parent/guardian/sibling. This form is not to be used for Holiday suspensions. Holiday suspensions can be submitted at http://www.aquazone.com.au/membership-suspension 

How do I request a credit/suspension?

  1. Fill out the request form below

  2. Our membership software requires a start and finish date for the suspension period, to process a credit/suspension.

  3. We will send you an email to confirm that credit has been applied or your membership has been suspended.

  4. If you have not received a confirmation email within 10 business days please email aquazone@warrnambool.vic.gov.au to enquire on the status of your request.

Please note that your request is not approved until the confirmation email is sent, which may be within 5-10 business days of receipt.

Terms & Conditions

1. I understand all requests are subject to approval by management.

2. I understand I may be asked to provide a doctor's certificate to support my request for medical suspension.

3. I understand medical suspensions may not exceed 8 weeks, unless prior approval by management.
